MAN: Hi there. I was wondering if I could see a doctor today? I've been having this persistent cough for about a week now.
WOMAN: Good morning. Yes, we do accept walk-in patients. Have you been to this clinic before?
MAN: No, I just moved to the area last month. I haven't had a chance to find a family doctor yet.
WOMAN: I see. Well, welcome to the neighborhood. I'll need you to fill out some new patient forms. Do you have your health card with you?
MAN: Yes, I do. Here it is. Um, how long do you think the wait will be? I have a meeting at work in about two hours.
WOMAN: Let me check... We have three doctors on duty today, and there are currently five patients ahead of you. I'd estimate about an hour wait, but it could be a bit longer depending on the complexity of each case.
MAN: Hmm, that should be okay. Is there a pharmacy nearby in case I need to fill a prescription afterward?
WOMAN: Yes, there's one right next door. They're open until 9 p.m. Here are the forms. Please fill them out completely and bring them back to me when you're done.

- Question 5, read aloud
Question 5: listen, then show the question text
How much does a doctor's note cost?
Transcript (read it after you answer)
MAN: I've finished the forms. Here you go.
WOMAN: Thank you. Let me just review these quickly... Everything looks good, but I notice you've left the emergency contact section blank.
MAN: Oh, right. I don't really know anyone in town yet. I guess I could put my sister's information, but she lives in Vancouver.
WOMAN: That's perfectly fine. Distance doesn't matter for emergency contacts. We just need someone we can call if necessary.
MAN: Okay, let me add her details then. Actually, while I'm waiting, is there any way I could get a doctor's note for work? I've missed two days because of this cough.
WOMAN: You'll need to discuss that with the doctor, but yes, they can certainly provide a note if they feel it's appropriate. There is a $25 fee for doctor's notes, though, just so you're aware.
MAN: That's fine. My employer will reimburse me. Oh, and one more thing – do you know if my prescription coverage starts immediately with my new health card?
WOMAN: Your health card covers the doctor's visit, but prescription coverage depends on your private insurance plan. Do you have insurance through your employer?

MAN: The doctor suggested I get a chest X-ray. Do I need to go to the hospital for that?
WOMAN: No, actually there's a diagnostic imaging center just three blocks from here. Let me print out the requisition form and directions for you.
MAN: Great, thanks. Do I need to make an appointment, or can I just walk in there too?
WOMAN: For X-rays, they usually accept walk-ins, but there might be a wait. If you'd prefer, I can call and schedule an appointment for you. They're open until 6 p.m. today.
MAN: That would be really helpful, thanks. I'd like to get this done as soon as possible. Um, also, the doctor mentioned something about coming back for test results?
WOMAN: Yes, the results are usually ready within 48 hours. The doctor would like to see you for a follow-up appointment to discuss them. Would you be available this Thursday afternoon?
MAN: Thursday should work. What time?
WOMAN: We have an opening at 2:15 p.m. And I've just confirmed with the imaging center – they can take you at 4:30 today. Does that work for you?
MAN: Perfect. I really appreciate your help with all of this.
